Our community outreach services provide support to enable people to live independently in their own homes or with their families.
Our community outreach services provide support to enable people to live independently in their own homes or with their families. Services are designed to meet the specific needs of each person and cover a wide range of personal and social care needs. For example, we can provide help around the home, such as assisting people with cooking nutritious meals, cleaning and learning how to budget. We could be helping them with aspects of personal care such as taking medications on schedule. Or it might mean support them as they get involved in activities out in the community, such as attending college, working, volunteering or taking part in hobbies and sports.
Our skilled outreach carers will also encourage clients to think about their own skills and strengths and how they could use them to contribute to their local community, allowing them to engage with those around them and develop a greater sense of citizenship.
Support can be provided daily, seven days a week, with a minimum of two hours a day for each day care is required. More support can, of course, be provided where needed and if agreed funding or individual budget allows.
